At FunctionSmart Physical Therapy, we accept most PPO plans and some HMO plans that have contracted with us. We will work with you to determine insurance coverage prior to your first visit so you will know what your coverage is. Here are just a few of the major insurance plans we accept:

• CPMG for pediatric bowel/bladder issues only

• Self-Pay 

For assistance with insurance questions, please call or e-mail our insurance specialist. For anyone who is out of network or seeking services that are not covered by the terms of their plan, we do have self pay options and will provide current rates on request prior to scheduling. We will also provide a "Good Faith Estimate" for the evaluation if requested and for any follow up care once the evaluation has been completed.
